For the majority of residents, admission to Interfaith House is their first opportunity to access a broad range of critical resources. Interfaith House has over 100 linkages with organizations in our community that support our clients recovery each step of the way.
Health Services Collaborative
We have forged partnerships with neighboring agencies to provide the best care possible, including:

- P.C.C. Community Wellness Center
Committed to maximizing the care and treatment residents receive during their stay, Interfaith House maintains an on-site health clinic that, thanks to our committed health collaborative partners, offers primary care services five days a week.
Behavioral Health Collaborative
Interfaith House works in association with the Illinois Office of Mental Health and Mt. Sinai Hospital to improve access to mental health services. We also partner with the Illinois Office of Alcoholism and Substance Abuse and other area providers to develop an innovative approach to serve those struggling with substance abuse issues.
Chicago Housing and Health Partnership
In January 2002, with Interfaith House acting as lead agency, a multi-disciplinary group of health care, respite care and housing providers began meeting to better serve homeless individuals with chronic medical health conditions by forming a new Chicago collaboration, the Chicago Housing and Health Partnership. Our goal is to increase health outcomes while stabilizing long-term housing. Project partners include:
